RN Hospice Case Manager

We provide an individualized orientation and training program!\n\nSamaritan is looking for a caring and experienced Registered Nurse Case Manager to deliver high-quality, patient-focused Hospice Care. You will manage a group of patients, making sure they receive expert care focused on comfort, managing their pain, and improving their Quality of Life. We give every new team member special one-on-one training when they start.\n\nThis role pays an hourly wage of $32.26 to $43.68, depending on your years of experience.\n\nKey Responsibilities\n

• Check on patients regularly and assess them for comfort, focusing on pain and symptom relief.\n
• Create and carry out a personal Care Plan tailored to what each patient needs.\n
• Teach patients and their families about Hospice Nursing and what to expect with their care.\n
• Work quickly to get the necessary Physician Orders to manage symptoms effectively.\n
• Plan and organize your home visit schedule efficiently, keeping track of your time and mileage.\n
• Connect patients with other members of the Interdisciplinary Team (IDT), like social workers, and work with them to meet the patient's full range of needs.\n
• Provide total, whole-person care that supports the patient's and family's physical, emotional, and social well-being.\n\nRequirements\n
• Must have an active Registered Nurse (RN) license in New Jersey.\n
• At least two (2) years of recent work experience as a nurse.\n
• A Bachelor of Science in Nursing (BSN) is preferred.\n
• The ability to speak a second language (Bilingual) is a bonus.\n\nCompensation and Benefits\n
