No RTO will be approved during the first week of the assignment or during holidays.

All facilities the traveler is open to working at must be included on the submission.

If willing to work “any location,” please specify (e.g., Any AZ, Any CO, Any state, etc.).

All shifts the traveler is open to must be documented.

All departments the traveler is willing to work must be checked on the cover page.

All candidates must be pre-closed and ready to accept if offered.

Please notify us immediately if the traveler withdraws or becomes unavailable.
