At our 50-bed hospital, we provide specialized inpatient and outpatient rehabilitative services to our patients, serving Stockton and surrounding areas. We see patients who often are recovering from disabilities caused by injuries, illnesses, or chronic medical conditions.

We are seeking a Case Manager to join our team of passionate patient caregivers!

Responsibilities

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and / or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital’s mission and “Guiding Principles” into daily practice.

The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
