About the Role :

Provides direct services by means of education, advocacy and services coordination to individuals injured through or exposed to violence in order to extend the psychosocial support services of the Center for Hope Violence Intervention Programs, in collaboration with the MONSE Safe Streets program, PAAVE (Pathways to Advocacy Against Violence Everyday) and other Center for Hope Programs. POSITION LOCATED AT GRACE MEDICAL CENTER.

Key Responsibilities :

Meets victims and loved ones following violent injury, assesses social and psychosocial barriers and safety and retaliation risks associated with the patient.

Maintains information on available community resources, makes appropriate and timely referrals for services and provides ongoing coordination of needed services.

Requirements :

Salary requirements $20 to $24 / hour based on experience and internal equity for grant funded position
