Description :
As a member of the interdisciplinary team, this role contributes professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process.
The nurse promotes the use of evidence-based practice through the nursing process, protocols, and procedures.
The role involves assessing the patient's status through interviews, observations, and physical assessments of actual problems and outcomes.
An individualized plan of care is developed using a collaborative approach with the patient / family and health care team.
The nurse uses appropriate interventions identified in the plan of care to achieve expected patient outcomes.
Ongoing evaluation and documentation of the patient's progress toward expected outcomes / goals occur throughout the shift.
The plan of care is modified based on evaluation results.
Required documentation, including patient charts and event reports, is completed as appropriate.
Professional accountability for one's own practice is demonstrated through seeking resources and continuing self-development.
Technical knowledge of equipment is required, along with troubleshooting skills when problems arise.
Current knowledge and skills in evidence-based nursing practice must be acquired and maintained.
The nurse promotes the professional development of self and team members.
Clinical knowledge is shared through normal teaching and in response to the learning needs of others.
A practical problem-solving approach to patient moving and handling issues is essential.
Skills to promote patient and staff safety through appropriate use of lift equipment are necessary.
Knowledge and skills to obtain adequate lab specimens for testing are required.
Patient bedside lab testing and associated quality control are performed in compliance with medical provider's orders and hospital procedures / policies.
The nurse coordinates interdisciplinary care of the patient through collaboration with team members and utilizes additional resources when necessary.
Clinical situations are analyzed in a systematic way, and resources are obtained or problems corrected to meet or exceed expected outcomes or patient needs.
The ability to recognize the need for assistance and utilize resources, including physician and nurse consultations, is crucial.
Ownership and persistence to ensure problems are resolved in the best way possible are expected.
The nurse should be able to manage non-complex and some complex patient cases while adapting to turnover in assignments.
Work should be prioritized based on changes in patient condition, assignment changes, and unanticipated interruptions.
Team resources should be identified and utilized to solve identified problems or needs.
Collaboration with the health care team in organizing tasks and activities for patient care from admission to discharge is important.
The nurse works with team members in delegating tasks and activities appropriately.
Providing a goal-focused shift report is essential.
Advocacy with physicians on the patient's behalf when necessary is part of the role.
Effective communication and teamwork strategies aimed at promoting high team performance and achieving optimal patient outcomes and unit productivity are essential.
