Caregiver For Homebound Patients

We are seeking compassionate and skilled Home Health LPNs to provide personalized care in patients' homes. Our team values nurses and is committed to putting patients first.

The ideal candidate will have a graduate degree in practical nursing and current licensure as an LPN / LVN in South Carolina. At least one year or 1200 hours of experience as an LPN / LVN is also required.

Responsibilities :

Provide home-based care to patients with illnesses, injuries, and chronic diseases often after hospital discharge under the supervision of an RN.

Work with a team of physicians, RNs, CNAs, and aides to deliver quality patient care.

Taking patient vitals, do assessments, document progress, and report changes to RN supervisor.

Assist RN in carrying out patient's plan of care and teaching patient self-care techniques.

Educate family on the patient's plan of care, medication, and home safety.

Collaborate with interdisciplinary team to coordinate patient care and plan for discharge.

Requirements :

A graduate degree in practical nursing (LPN / LVN).

Current license as an LPN / LVN in South Carolina.

At least one year or 1200 hours of experience as an LPN / LVN.
