American Traveler is seeking an experienced RN Case Manager for a primary care clinic position in Alaska, requiring an active AK RN license and recent case management experience.
\n \n Responsibilities\n
Position based in a primary care clinic settingDay shifts, Monday through FridayInvolves management of both acute and chronic care patient needsCoordination of care for high-risk and complex patientsParticipation in multi-disciplinary care conferences and case reviewsCoordinates specialty clinic referrals and manages waitlistsHandles OB case management, including tracking and education for OB patientsFacilitates video teleconferencing visits and coordinates exercise treadmill testsResponsible for coordinating and managing care for SEARHC patients across different programsActs as the primary link for care coordination, tracking referrals and outcomesProvides patient education, outreach, and support as an advocate for individuals and familiesParticipates in clinic team improvement efforts and serves as a leader among clinical staffPerforms staff nursing duties including assessment, documentation, and medication administrationMaintains compliance with safety, infection control, and annual competenciesWorks collaboratively as part of the clinical and interdisciplinary teamMust maintain a clean and safe work environment and report safety hazards as needed\n \n Requirements\n
Active, unrestricted AK RN license required at time of considerationGraduation from an accredited school of nursing; BSN preferredAt least 1 year of case management experience within the past 2 yearsBasic Life Support (BLS) certification requiredAbility to work independently in a fast-paced clinical environmentMust provide proof of flu vaccinationFingerprint cards required prior to start, with a start date at least 2 weeks after receipt\n Benefits\n
