Job Description\n

American Traveler is seeking an experienced RN Case Manager for a primary care clinic position in Alaska, requiring an active AK RN license and recent case management experience.

\n \n Responsibilities\n

  • Position based in a primary care clinic setting
  • Day shifts, Monday through Friday
  • Involves management of both acute and chronic care patient needs
  • Coordination of care for high-risk and complex patients
  • Participation in multi-disciplinary care conferences and case reviews
  • Coordinates specialty clinic referrals and manages waitlists
  • Handles OB case management, including tracking and education for OB patients
  • Facilitates video teleconferencing visits and coordinates exercise treadmill tests
  • Responsible for coordinating and managing care for SEARHC patients across different programs
  • Acts as the primary link for care coordination, tracking referrals and outcomes
  • Provides patient education, outreach, and support as an advocate for individuals and families
  • Participates in clinic team improvement efforts and serves as a leader among clinical staff
  • Performs staff nursing duties including assessment, documentation, and medication administration
  • Maintains compliance with safety, infection control, and annual competencies
  • Works collaboratively as part of the clinical and interdisciplinary team
  • Must maintain a clean and safe work environment and report safety hazards as needed

  • Active, unrestricted AK RN license required at time of consideration
  • Graduation from an accredited school of nursing; BSN preferred
  • At least 1 year of case management experience within the past 2 years
  • Basic Life Support (BLS) certification required
  • Ability to work independently in a fast-paced clinical environment
  • Must provide proof of flu vaccination
  • Fingerprint cards required prior to start, with a start date at least 2 weeks after receipt
