Voca Job ID#17290938; Shift : 0800-1700; Would work remote (out of their home) to start the day and then would go into the field; would be traveling to patient homes and LTC facilities and Will serve Assisted Living and SNF's; AHA BLS cert required; NC or compact license required; Required skills : Experience with Homecare Homebase is required; Experience working Hospice Home Health, as well as visiting patients on hospice care in Long Term Care settings; Catheter insertion and care, feeding support to include g-tubes, CAD pumps for pain control, medication management, psychosocial support and strong listening / communication skills are needed in this role, assessment of patient / family needs, bereavement support, ability to teach families and patients on EOL and expected decline throughout the EOL process; NC or compact license required
