HOME HEALTH LPN | DETAILS AND COMPENSATION :
- Location : Virginia Beach VA 23452 – 100% On-site
- Payrate : $37.30 / hr
- Required Availability : Full-Time | Monday–Friday, 8 : 00 AM – 5 : 00 PM
(on-call and overtime may be required occasionally)

The Home Health LPN role involves providing high-quality patient care, supporting treatment plans, and ensuring compliance with medical and safety standards.
This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needsBenefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insuranceA 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility datesHOME HEALTH LPN | RESPONSIBILITIES :
Provide care in alignment with the patient’s individualized plan of care, while maintaining dignity and independence.Monitor vital signs and promptly report abnormal findings.Administer medications, feedings, oxygen, and ostomy care as directed.Support therapy routines established by Physical, Occupational, and Speech Therapists.Maintain infection control protocols and report potential communicable diseases or infections.Document care provided, progress toward goals, and patient outcomes accurately and thoroughly.Order necessary supplies, medications, and feedings to maintain adequate stock.Respond to patient incidents, complete incident reports, and ensure appropriate follow-up.Ensure medical equipment is functioning properly and report any issues for repair.HOME HEALTH LPN | REQUIREMENTS :
Education : High School Diploma or GED (Verifiable).Attendance : Commitment to mandatory attendance during the first 90 days.Active Licensed Practical Nurse (LPN) license (required).Wound care and IV experience (required).HCHB (HomeCare HomeBase) experience (required).Strong bedside manner and communication skills.Take the Next Step!
