Position : Registered Nurse (RN) – Case Manager (Home Health / Hospice)

Location : Fond du Lac, WI and surrounding counties

Start Date : ASAP

Contract Length : 13 weeks (with potential to extend)

Guaranteed Hours : 40 hours / week

Shift : Monday–Friday, 8 : 00 AM – 4 : 30 PM

Weekend / On-Call : None

Serve as a Case Manager supporting home health and hospice patients across Fond du Lac , Dodge , and Columbia counties. This is a field-based role, requiring travel between patient homes and occasional facility visits. The RN will perform comprehensive assessments, coordinate care plans, collaborate with interdisciplinary teams, and manage end-of-life symptom support for patients and families.

Coverage area includes Fond du Lac, Dodge, and Columbia counties

Mileage reimbursed at $0.53 / mile

EMR : Home Care Home Base (preferred); MatrixCare or other EMR acceptable

Candidates without HCHB or hospice experience will be trained at a reduced bill rate until competencies are completed

Training hours required will vary based on prior experience; typically ~16 hours for HCHB

Required skills checklist and compliance documents must be uploaded before start
