Description : Summary

Under the supervision of the Director of Ibero-American Action League, WNY, the Community Health Worker WNY serves as a key point of contact for individuals seeking assistance from Ibero Niagara Falls. This role involves screening, assessing, and providing referrals to Medicaid-eligible clients who may need assistance with housing, food, health care, transportation, or other essential human services. This role will actively provide appropriate community agency referrals as well as perform various clerical duties. Qualified candidate will speak and write in English and Spanish. MUST be Bilingual in Spanish and English, both written and verbal .

Operational Services :

• Screen and assess clients to determine their needs and eligibility for assistance for housing, food, health care and other essential human services.
• Provide services in English and Spanish, written and verbal, as needed.
• Complete the intake process using a standard questionnaire form.
• Collect and maintain accurate client information and documentation.
• Perform clerical tasks, including data entry, filing, and maintaining organized records.
• Based on intake forms, determine and provide appropriate community resource referrals as needed for health care, food, clothing, and / or housing.
• Serve as the initial point of contact by answering all incoming calls promptly and courteously.
• Greet clients and visitors professionally, ensuring a welcoming environment.
• Collaborate with other team members to deliver seamless client support and maintain good working relationships with staff from funding sources, related agencies, and other Ibero programs.
• Provide information regarding program location and agency services to callers and visitors.
• Verify that the referred individual meets program requirements including having an active Medicaid case.
• Assist community members who need help applying for any benefits and entitlements, Section 8, Department of Motor Vehicle forms, Unemployment, and Job search as needed.
• Safeguard confidential information.
• Maintain sign-in-out logbooks for all visitors.
• Be available to work outside regularly scheduled working hours when needed.
• Participate in related agency and community activities.
• Run EPACES for Adult Health Home clients the first week of every month to ensure their Medicaid is active.

Authority :

This position holds a relatively high degree of independence within the framework of the agency’s policies and procedures. Must exercise considerable autonomy and initiative and make decisions regarding completion of tasks and monitoring activities. Must use sound judgment in reaching those decisions and use established policies and procedures in the day-to-day direct service tasks.

Physical Demands :

The position does require occasional standing, squatting, and lifting to approximately 40 lbs. And frequent sitting.
