Physical Therapist (Inpatient)

SUMMARY

The Physical Therapist (PT) Inpatient position is responsible for the provision of Physical Therapy services for an inpatient and / or outpatient population from age 15 and older including, but not limited to, evaluation, evidence-based intervention, equipment recommendations and modality application, under the referral of a physician or qualified non-physician practitioner.

ESSENTIAL FUNCTIONS

• Administers PT evaluations to inpatients and outpatients and determines appropriate treatment programs and / or equipment needs, integrating age specific needs and issues for physical medicine rehabilitation population.
• Develops a patient-centered treatment plan based upon the evaluation, patient goals and age-related issues / needs to optimize treatment and function.
• Responsible for assuring the treatment plan supports medical necessity and incorporates evidence-based treatments and outcome measures.
• Responsible for ensuring the treatment plan is revised / updated appropriately per policy, communicated to appropriate team members including attendance at team conferences, and reviewed and signed off by referring physician (as appropriate).
• Charges appropriately for services provided and appropriately documents outcome measures.
• Provides clinical supervision, including but not limited to coordination of care and delegation of duties, to assistants, techs, and students according to state / federal regulations and hospital guidelines.
• Other duties as requested.
