What Youll Do in This Role :

Bring your passion for people and service to a team that makes a real difference every day! As a Day Center Concierge in our PACE (Program of All-Inclusive Care for the Elderly) center youll be the welcoming face and trusted support for participants helping them maintain their independence dignity and quality of life.

Youll provide compassionate person-centered care in a vibrant day center setting ensuring every participant feels valued and cared for. Working alongside our interdisciplinary PACE team youll help deliver seamless holistic support while maintaining the highest standards of safety quality and respect.

Primary Responsibilities :

  • Provide homemaking meal preparation medication reminders and respite support to participants in their home environment and in the day center environment as needed.
  • Provide reminders prompts and cuing for ADL and IADL tasks as outlined in the participants plan of care.
  • Assistance with transportation needs including bus travel and ride-along.
  • Riding scheduled routes can be AM or PM routes
  • Provide support to the driver for loading and unloading participants
  • Door-to-door service excellence
  • Regularly collaborate with members of the interdisciplinary team (IDT) to ensure participants service needs are met.
  • Communicate effectively with other team members to share observations updates or concerns regarding participants conditions.
  • Accurately document all services and observations in participants medical records in compliance with organizational policies and regulatory requirements.
  • Ensure a safe and clean environment for participants in both home and day center settings.
  • Assist with light housekeeping tasks and meal preparation when needed.
  • What Youll Need :

  • At least one year of experience working with frail elderly or long-term care populations.
  • High school diploma or GED required
  • Reliable transportation and a valid Michigan Drivers License required.
  • Must possess current CPR certification or willing and able to obtain as offered by LifeCircles.
  • Ability to lift 35 pounds without assistance
