LPN CHRONIC CARE MGT SPECIALIST / INTERNAL MEDICINE

Monday - Friday, 4 days / wk; Pay rate starts at $20.00 / hr

JOB REQUIREMENTS

EDUCATION

Minimum : High School diploma / GED and current IN Practical Nurse Licensure.

LICENSE / CERTIFICATION

LPN license from the state of Indiana required.

CPR required.

EXPERIENCE

Preferred : At least one year LPN experience in physician practice setting.

SUMMARY

• Demonstrates customer service through teamwork creating an efficient environment for patients and staff.
• Coordination of Care Transitions
• Promotes safe quality patient care through laboratory services, medication documentation, and admission and discharge process documentation.
