About the Opportunity :
Pair Team is hiring a full-time Bilingual RN Care Manager to support a high-need Medicaid population through proactive, whole-person care. This role is part of Pair Team's Care Management Program, which delivers integrated medical, behavioral, and social support to individuals with complex needs.
The RN Care Manager will serve as the primary point of contact for a panel of high-risk members, contributing clinical expertise to build trust, address health disparities, and coordinate care across settings. This role plays a central part in an interdisciplinary care model, working alongside Community Health Workers, Behavioral Health Care Managers, and Nurse Practitioners to improve outcomes for individuals with chronic conditions, behavioral health needs, and / or significant social barriers to care.
This position primarily allows for remote work; however, it includes regular in-person visits in the community across Travis County (Austin, TX) . In-person activities typically occur 1-2 times per week, while the majority of duties are performed from home.
