Description :

This position involves working 2 days at one facility and 3 days at another site.

Candidates must ensure the necessary documentation is completed and attached upon submission.

The submission should include a resume and license verification.

If these items are not included, the candidate may not be considered.

Candidates will undergo a thorough credentialing checklist that includes state license verification, references, and background checks.

Requirements also include a Multi-State Criminal Search, Nationwide Sex Offender Search, and HHS / OIG Search.

A Social Security Number trace is required as well.

Candidates must also provide proof of a 2-Step PPD or Quantiferon test, or proof of a positive PPD along with a chest X-ray.

This position has a normal urgency for filling.

No holiday coverage is required.
