We are actively seeking a mission-driven Mental Health Counselor (LMSW or LMHC) to join a supportive housing program in the Bronx. This is a meaningful opportunity to work with a renowned NYC non-profit organization dedicated to improving the lives of homeless, at-risk, and mentally ill individuals.

Mental Health Counselor Key Responsibilities :

• Provide ongoing case management, clinical support, and care coordination to residents in supportive housing
• Conduct psychosocial evaluations, develop service plans, and assess progress toward recovery and independence
• Deliver short-term counseling, crisis de-escalation, and wellness interventions tailored to client needs
• Assist clients in accessing public benefits, healthcare services, and community-based supports
• Facilitate educational and therapeutic groups focused on recovery, health, and independent living skills
• Maintain timely, accurate, and compliant documentation aligned with organizational and funding guidelines
• Collaborate with hospitals, providers, and community agencies to ensure continuity of care
