Must be within 50 miles

Provider s own office locations- Provider must have their own office

Grayson- 2 days per week needed

• Ashland- 1 day per week needed
• Campbellsville- 2 days per week needed
• Pineville- 2 days per week needed

PRN as needed.

1099 compensation :
