GENERAL STATEMENT OF DUTIES
Elbert County Public Health Department is seeking a compassionate, dedicated, and highly skilled Nurse Practitioner (NP) to provide primary and preventive health care services to individuals and families across their lifespan. The NP will function as an integral part of a multidisciplinary team to deliver high-quality care in a public health setting, with a strong focus on underserved and vulnerable populations.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conduct comprehensive health assessments including physical exams, medical histories, and screenings
- Diagnose, treat, and manage acute and chronic health conditions
- Order, perform, and interpret diagnostic tests and lab results
- Prescribe medications and non-pharmacologic treatments in accordance with state laws and collaborative agreements
- Provide patient education on disease prevention, healthy lifestyle choices, and chronic disease management
- Collaborate with physicians, nurses, case managers, social workers, and public health staff
- Participate in health promotion and disease prevention initiatives
- Maintain accurate and up-to-date electronic health records (EHR)
- Provide oversight and assist in developing clinic and public health policies and procedures
- Adhere to clinical protocols, policies, and standards of care
- Participate in quality improvement, data reporting, and public health research initiatives as needed
- Support emergency response efforts and community outreach activities
- Maintain compliance with all HIPAA and confidentiality requirements
- Completes job specific training as required
- Various additional duties as assigned

Master’s Degree in Nursing (MSN) from an accredited programCurrent, unrestricted Nurse Practitioner License in ColoradoCertification as a Family Nurse Practitioner (FNP)Basic Life Support (BLS) certificationCurrent DEA license and prescriptive authority (or ability to obtain)Department of Transportation (DOT) Certification - preferredA minimum of 1-2 years supervised experience as a nurse practitioner is requiredADDITIONAL REQUIREMENTS
Valid Colorado Driver’s LicenseAcceptable Motor Vehicle RecordMust be able to pass a criminal background check and drug testBilingual a plusPHYSICAL REQUIREMENTS FOR POSITION
