Job Summary : JOB SUMMARY

Assists the nursing staff and patients with basic care needs, implementing orders, and evaluating care for various patients within the facility. Reports directly to the charge nurse and Clinical Director of the unit. This position works in conjunction with other members of nursing service, ancillary services, physicians, patients, and families. It collaborates with various members of the healthcare team to ensure quality of care across the continuum. Requires the ability to be self-directed and quality focused. Interpersonal communication skills are a necessary function of this position.

POSITION HOURS

Three (3) 12-hour shifts

6 : 45 a.m. - 7 : 15 p.m.

Rotating (including weekends & holidays) based on unit need.

No specific workdays or days off can be guaranteed.

DEPARTMENT SUMMARY

In the One West (1W) unit, adult patients are treated under an Observation status (stay

Care Partners with experience in palliative care, hospice, and nursing home care are encouraged to apply.

Qualifications :

Mandatory : High School Diploma or GED (or equivalent experience) and one of the three below :

• Certification : Graduation from a Nursing Assistant or Aide Training Program, (the certification is not mandatory, but a certificate of completion is necessary) or
• Experience : Minimum of six (6) months of previous experience as a care partner or nursing assistant / technician within the last two (2) years or
• Education : Current enrollment in a program of professional nursing LPN or RN and successful completion of the first semester of nursing school to include nursing fundamentals
