Position Summary :
As a Case Manager, you will perform case management according to the Gap Housing Program, which offers preventative and homeless services. The program provides short-term transitional housing for one (1) to fourteen (14) days before an individuals or familys official move-in date. In this role, you will be responsible for the program guidelines, eligibility, intake, duration criteria, compliance, referrals, and coordination with housing vendors who have agreements with the firm.
Position Requirements
- Develop written program guidelines that specify eligibility criteria, enrollment procedures for the housing program, length of stay, referral process, and client responsibilities.
- Provide case management in collaboration with the Citys Community Outreach Specialist, including assessment, intake, and referral support.
- Coordinate transportation to and from temporary housing, establish contact with housing providers, and deliver daily case management services to clientsincluding individuals and familiesuntil they are placed in permanent housing.
- Offer ongoing case management for each consenting client to support stabilization and transition into permanent housing.
- Provide proactive case management services, which may include, but are not limited to, enrollment in housing programs or placement on affordable housing waitlists (such as AMI-based apartments, rehousing programs, etc.).
- Offer referrals to substance use treatment, mental health services, employment or workforce programs, and other supportive services as needed.
- Perform other duties as assigned.
Required Education and Experience
