Facility Information

6 Exam rooms :

• 1 procedure room
• 2 isolation rooms
• 1 pediatric room
• 2 standard exam rooms

Pt. seen per day :

Support staffing : 2 3 clinical support staff per shift. Mix of RN s & CMA s

Medical Director available via email / EPIC / phone during the day

Patient Population : All ages

Job Details

Type : NP only, PA s cannot be considered

New Grads Accepted : No, must have experience and be able to work independently

Clinic Schedule : 8am- 6pm four days a week, no call

EMR : EPIC

Licenses and Certifications : Oregon medical license and DEA, BLS
