Primary Care Physician (Internal Medicine, Family Practice, or Geriatrics)

Job Overview :

We are seeking a dedicated Primary Care Physician to join our team, focusing on comprehensive care for elderly participants in our PACE program. This role involves working closely with an interdisciplinary team to help seniors maintain their independence and quality of life in their communities.

Key Responsibilities :

• Conduct patient assessments and develop individualized care plans.
• Oversee patient care in nursing homes and hospitals, managing admissions and discharges.
• Provide leadership and guidance to Nurse Practitioners and other team members.
• Participate in rotating night and weekend on-call duties.
