Pact Lpn Role

The PACT LPN is responsible for supporting and optimizing self-care management for PACT patients. This role involves exceptional collaboration and communication with the Core Team (including the Patient, PACT Provider, RN Care Manager, and Clerk) as well as the Expanded Team (family / caregiver) and other internal and community-based services involved in patient care. The goal is to ensure timely access, comprehensive services, and effective coordination to enhance the patient's overall care.
