Diabetes Coordinator

POSITION SUMMARY :

The Diabetes Care and Education Coordinator is a vital link to keeping high-risk patients engaged to improve their health outcome . Will provide healthy lifestyle education and coordination of care for high-risk diabetes patients.

Supervision Exercised :

This position has no supervising duties.

Supervision Received :

Reports to the Pharmacy Programs Manager

ESSENTIAL JOB RESPONSIBILITIES :

• Will be responsible for accepting referrals from primary care teams and scheduling diabetes patients for educational services
• Assist diabetes patients with obtaining continuous glucose monitors, blood glucose meters and training patients on appropriate use
• Providing patients with healthy lifestyle education and setting goals for management of diabetes
• Monitoring DRVS for diabetes measures and goals
• Identifying barriers to diabetes goals and care and assist with navigation of those barriers
• Help create and grow diabetes education program within the facility
• Provide support to primary care and nursing staff
• Other duties as assigned.

MINIMUM QUALIFICATIONS :

High School Graduate or GED

CPR certified

Background in the medical field

Demonstrate knowledge of health education subjects

Ability to complete forms in an orderly and accurate manner

Able to use sound judgment . Possess a calm disposition and the abil ity to work easily with people

Ability to relate to the public in all areas
