Position Overview

This position is 100% mobile you ll travel to patients homes and provide care directly in their living environments. This role is ideal for one who values autonomy, continuity of care, and building long-term patient relationships.

Key Details

• Setting : Home-based primary and palliative care
• Schedule : Patient visits throughout the week, with 1 2 administrative days for documentation, planning, and case reviews
• Patient Load : Approximately 150 visits per month , with flexibility based on patient needs
• Compensation : $115,000 base salary with additional earning potential for exceeding visit expectations
