Join our team at Prisma Health, where you'll assess, develop, implement, and evaluate nursing care for patients in their homes. You will play a critical role in coordinating with various disciplines and agencies to provide comprehensive patient care.\n\nThe ideal candidate will have experience managing complex chronic conditions, focusing on preventative care, patient education, and proactive interventions to enhance patient outcomes and minimize emergency room visits and hospital readmissions. • Communicate effectively with interdisciplinary teams to ensure optimal patient care and agency operation.\n
• Thoroughly document patient assessments, interventions, and care plans from admission through discharge.\n
• Maintain professionalism while collaborating with medical staff on patient care.\n
• Regularly assess patients, adjusting care plans based on their evolving needs.\n
• Proactively plan for discharge and address ongoing care requirements.\n
• Complete and submit documentation accurately and on time.\n
• Contribute to quality improvement projects and participate in audits.\n
• Be flexible in accepting assignments outside of regular duties as needed.\n
• Perform additional duties as assigned.\n\nMinimum Requirements\n
• Education : Associate degree in Nursing.\n
• Experience : One year of experience as a Registered Nurse.\n\nRequired Certifications, Registrations, Licenses\n
• Valid Registered Nurse license in South Carolina.\n
• BLS Certification.\n
• Valid driver's license and acceptable motor vehicle record, with proof of auto insurance.\n
