Superior Senior Care in Fort Smith is looking for an LPN, RN, or licensed Social Worker to perform in-home, physical-dependency assessments of ADL’s and IADL’s for elderly and disabled clients in Sebastian, Crawford, Franklin, Logan, and Scott counties. Initial assessments, reassessments, supervisory visits, aide skills check, aide record review are required per the Arkansas Medicaid Rules and Regulations.

The TCM nurse or social worker must be organized, self-motivated and detail-oriented to succeed at this position. Must be

1. Licensed in the state of Arkansas as a social worker (Licensed Master Social Worker or Licensed Certified Social Worker), a registered nurse or a licensed practical nurse;

2. Have a bachelor’s degree from an accredited institution in a health and human services or related field; or

3. Have two years’ experience in the delivery of human services, including without limitation having performed satisfactorily as a case manager for a period of two (2) years (experience must be within the past three (3) years). You also must have reliable transportation and be willing to travel throughout our service area. Case management experience is a plus.

Schedule is very flexible; sets own appointments.
