We are seeking a dedicated Nurse Practitioner to deliver comprehensive primary care services to residents of assisted living facilities. The ideal candidate will have experience in geriatrics and / or long-term care, committed to improving the quality of life for older adults.

In this role, you will conduct thorough health assessments, develop personalized care plans in collaboration with an interdisciplinary team, manage chronic and acute conditions, and provide education to residents and their families. This position offers flexible full-time or part-time schedules with no nights or weekends, ensuring a healthy work-life balance in a beautiful, supportive environment.

Job Requirements

Requirements

• Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program
• Unrestricted New York State Nurse Practitioner license
• Valid DEA permit to prescribe medications commonly used in practice
• Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid
• Minimum of 3 years clinical experience, preferably in long-term care or geriatrics
• Experience with eClinicalWorks (eCW) electronic health records preferred
