Job Summary :

As a Physical Therapist Home Health, you will be responsible for providing physical therapy services to patients in their homes, focusing on rehabilitative care, wound management, and maintenance of physical function. You will work closely with patients, families, and healthcare teams to develop and implement personalized treatment plans that promote optimal physical function, independence, and overall well-being.

Responsibilities :

Conduct comprehensive physical therapy assessments and evaluations

Develop and implement individualized treatment plans

Provide physical therapy services, including hands-on treatment, education, and training

Collaborate with patients, families, and healthcare teams to ensure continuity of care

Maintain accurate and detailed records of patient progress and treatment plans

Communicate effectively with patients, families, and healthcare teams to ensure effective care delivery

Requirements :

Certificate in Physical Therapy (PT)

1+ years of experience in home health or similar setting

Valid state physical therapy license
