Productivity Expectation : 27–30 points / day based on territory
Job Description :
We are seeking a dedicated and skilled Home Health Physical Therapist (PT) for an immediate opportunity with a respected home health provider in Washington State. This position offers an ideal schedule, supportive systems, and flexible patient scheduling, making it a great fit for both experienced travelers and local clinicians.
Position Highlights :
Caseload : Adult patients only
Daily Visits : 4–5 per day
Mileage Reimbursement : IRS standard rate
Scrubs : Any color permitted
Charting System : Home Care Home Base (HCHB) – Required
Documentation Equipment : Provided (must be returned)
Driving Radius : Occasionally up to 100 miles, though routes are grouped efficiently to minimize windshield time
Transportation : Must have own reliable transportation
Requirements
Required Experience & Qualifications :
Minimum 1 year of recent Skilled Home Health PT experience (within the last 3 years)
Inpatient-only experience will not be accepted
Current Washington PT License
BCLS Certification (Basic Cardiac Life Support)
Strong understanding of OASIS documentation
Must be comfortable working independently in home settings

Requirements
Responsibilities : - Provide direct patient care under the supervision of a Certified Medical Assistant or Licensed Practical Nurse or registered nurse. - Assist with activities of daily living, including bathing, dressing, and feeding. - Monitor and record vital signs, such as blood pressure, temperature, and pulse. - Assist with mobility and transfers, ensuring patient safety. - Follow aseptic techniques to prevent infection and maintain a clean and safe environment. - Assist with airway management and respiratory support as needed. - Collaborate with the healthcare team to develop and implement care plans. - Provide emotional support to patients and their families. - Document patient information accurately and timely.
