Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.
A skilled and compassionate Home Health Registered Nurse (RN) is needed to provide high-quality nursing care in patients homes. This role demands clinical expertise, strong communication, and the ability to work independently within a flexible, field-based schedule. You will perform comprehensive patient assessments, develop individualized care plans, and collaborate closely with interdisciplinary healthcare teams to ensure optimal patient outcomes.
Key Responsibilities :
- Conduct thorough OASIS assessments and create patient-specific care plans.
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify condition changes, and communicate promptly with healthcare providers.
- Provide education to patients and families regarding treatment plans, medications, and preventive care.
- Maintain accurate documentation using Home Care Home Base (HCHB) software.
- Collaborate with therapists, social workers, and home health aides for coordinated care delivery.
- Participate in case conferences, staff meetings, and continuing education.
- Adhere to all regulatory and agency standards for documentation and care quality.
Qualifications and Desired Experience :
Active RN license in the state of practice (good standing).Minimum 1 year recent home health experience, preferably field-based.Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.Strong clinical assessment and critical thinking skills.Reliable transportation, valid driver's license, and auto insurance.Effective communication, time management, and organizational abilities.BSN preferred.Experience managing a caseload of 2532 points per week is advantageous.Knowledge of Medicare and home health compliance standards.Schedule :
Full-time contract, 13 weeks.Monday through Friday, 8 : 00 AM 5 : 00 PM.Caseload typically involves 35 patient visits per day within an assigned territory.Location Insight : Raeford, North Carolina offers a warm, welcoming community with a blend of small-town charm and access to larger urban amenities.
