What you will do :
Conducts psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus.Contacts appropriate referral sources and / or previous or current tertiary care providers to inform of client's enrollment in treatment, obtain relevant collateral clinical data and coordinate care.Develops, implements and monitors progress of the patient's individualized treatment plan.Provides individual, couples and family process-oriented counseling sessions as appropriate using various professional treatment modalities.Participates in regular clinical team meetings and provides input regarding case discussions on the psychosocial, disease management and case management needs of patients.Demonstrates an understanding of, and adherence to, all procedures and legislation which apply to patients cared for in a Mental Health and / or Substance Abuse treatment setting.Informs attending physicians (psychiatrist, primary care physician or midlevel provider) in the incidence of psychotropic medication management, of client's treatment progress and / or significant changes in emotional / mental status, which undermine client's daily functioning.Petitions clients and effectively transitions them to inpatient care in the event of the need for psychiatric hospitalization.Demonstrates a comprehensive knowledge of community resources (12 step groups, support services, educational resources, etc.) to assure appropriate supplemental care or follow up services after termination.Demonstrates a comprehensive knowledge of community resources (inpatient treatment settings, recovery homes and residential treatment facilities) and medical allied staff expertise to assure appropriate transition to elevated levels of care for clients requiring more intensive treatment services.Maintains accurate and timely medical records including psycho-social assessments, family sessions and daily progress notes. Provides descriptions of therapeutic interventions and patient response for discharge planning process.What you will need :
