About the Role

As a Nurse Practitioner with Matrix, you’ll conduct various health assessments during in-home visits that last up to one hour. You will have the opportunity to :

• Perform adult and geriatric assessments, including medical history evaluation and treatment planning.
• Provide health education and facilitate referrals as necessary.
• Engage collaboratively with physicians, case managers, and family members.
• Implement care strategies directly in members' homes or Skilled Nursing Facilities.
