Job description

Nationwide Therapy Group is seeking a LPN / LVN Long Term Care for a travel job in Miller, South Dakota.

LPN - LONG TERM CARE

  • Shift : 12H Nights, every 3rd weekend
  • Description / Unit Notes : Good Samaritan
  • Items of Note :

    • Local rates apply within 80 miles o

  • No Charge Differential at Good Samaritan Facilities
  • No on call or call back available or billable
  • Backouts & quits with less than 14-days notice incur a $500 fee
  • Facility : GSS - Park River
  • Position : LPN
  • # Beds : 66
  • Ratios : 22-25 : 1 (currently experiencing low census)
  • EMR : PCC
  • Weekend Requirement : Every other weekend
  • Holidays : Every other holiday
  • Shifts (8s or 12s) : 8 hr.; currently have AMs and NOCs working 12s
  • Location specific orientation : Follow another nurse for 1 shift
  • Common diagnosis / Types of patients : CHF, dementia, DM, CKD, COPD, dysphagia (PEG tube), urinary retention (foley catheters), wounds
  • Notes entered : 4.18.25
  • Cancellation Policy : Client can give 2 week's notice
  • Charting System : PCC
  • BSN Required : False
  • License to Submit : Yes
  • Trauma Level : –
  • Care Setting : Post Acute
  • Division : Nursing
  • Shift Notes : 12 hour Nights with every 3rd weekend
