Care Central Professionals Home Health is seeking a compassionate and skilled Per Diem Physical Therapy Assistant (PTA) to join our team in Roseville, CA . This role offers flexibility and the opportunity to provide one-on-one care that helps patients regain independence and improve their quality of life in the comfort of their homes.

Responsibilities :

Provide physical therapy services under the supervision and direction of a licensed Physical Therapist (PT).

Carry out individualized treatment plans to improve mobility, strength, balance, and functional independence.

Educate patients and caregivers on safe exercise techniques, mobility strategies, and home safety.

Accurately document visits and patient progress in compliance with agency and regulatory standards.

Collaborate with the care team to ensure patient-centered, coordinated care.

Qualifications :

Current California PTA license in good standing.

Graduate of an accredited Physical Therapist Assistant program.

Minimum of 1 year of clinical experience (home health experience preferred).

Strong communication and interpersonal skills.

Valid driver’s license and reliable transportation.
