Position Highlights :

FTE : Full and part time options available; share with us your availability
Hours : Day Shift Monday-Friday, weekend and holiday rotation

Home Health : Libal Street Office, 1920 Libal St, Green Bay, WI 54301
Bellin Inpatient Rehab : 725 South Webster Green Bay, WI 54301
Bellin Hospital : 744 South Webster Green Bay, WI 54301
Oconto Hospital Swing : 820 Arbitus Avenue Oconto, WI 54153

Job Description :

Physical Therapy (PT) will conduct evaluations and patient care to improve functional mobility, reduce fall risk, increase strength, endurance, and independence to make a meaningful impact by improving patients' quality of life. PT will work collaboratively with the multi-disciplinary health care team and community resources to provide the best outcomes for patients.

This unique physical therapist role will cover patient care in multiple settings; including, home health, acute care, inpatient rehab, and swing bed.

Qualifications :

Wisconsin physical therapy license and CPR certification required (Bellin can help obtain during orientation); proof of current auto liability insurance and valid WI driver's license required.
