Job Description

The case management role is a critical link between providers, medical services, and social services for the individuals served. It aims to stabilize individuals across all life domains, including securing housing, increasing access to food, aiding workforce readiness and job searching, and coordinating medical, psychiatric, and other support services.

The case manager consults with professionals, family members, and others, incorporating their input into the Person-Centered Planning process. They inform stakeholders, monitor the recipient's progress toward treatment goals, and act as advocate, broker, coordinator, and monitor of the service delivery system. They coordinate movement across care levels, discharge planning, and community re-entry following hospitalization or residential services.

Effective case management can reduce crisis episodes, extend community tenure, and increase engagement in work, school, or social activities, leading to greater personal satisfaction and independence. Supports are tailored to individual needs, promoting residence in independent or semi-independent living arrangements and active participation in recovery.
