$2,500 to $2,550 per week

About The Position

Key Responsibilities :

Coordinate interdisciplinary care plans, ensuring alignment with patient needs and covered services.

Collaborate with physicians on resource utilization, treatment plans, length of stay, and discharge planning.

Manage transitions of care, including transfers to tertiary centers with complete medical documentation.

Conduct focused reviews for frequent ED visitors to identify trends and improve care delivery.

Monitor and report data for regulatory / accreditation compliance.

Partner with acute care case managers to optimize continuity of care.

Requirements :

2+ years of nursing experience  (REQUIRED).

6 months of home health experience  (REQUIRED).

Travel experience  (REQUIRED).

Active driver’s license, car insurance, and  Hawaii RN license  (REQUIRED).

Proficiency in  Homecare Home Base & OASIS  (REQUIRED).

COVID + Flu vaccination  (No declinations accepted).

Bachelor’s degree (Preferred).

Case management experience (Preferred).
