Reports to: Director of Nursing

Position Responsibilities Summary:

The Licensed Practical/Vocational Nurse si responsible for providing direct skilled client care under the
supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive client care outcomes.

Essential Job Functions/Responsibilities:

  1. Provide direct patient care as defined under the state nurse practice act using sound judgment and critical thinking.
  2. Implement plan of care and treatment initiated by the registered nurse.

  3. Provide accurate, detailed and timely documentation consistent with the plan of care and the nursing process.
  4. Conduct ongoing assessments.
  5. Participate in health/therapeutic counseling, teaching and emotional support to the client and family/caregivers.
  6. Observe client condition and accurately reports to the registered nurse.
  7. Assist the registered nurse with client discharge or transfer.
  8. Assess and provide client and family/caregiver education and information pertinent to diagnosis.
  9. Prepare, administer, and appropriately document actions taken specific or commonly prescribed oral, topical, subcutaneous and intramuscular medications. Observe, report and record
    adverse reactions to medications and/or treatment to the registered nurse.
  10. Recognize the need for and institute emergency measures in a rapid and appropriate manner, when indicated.
  11. Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (i.e., PT, O, ST, Dietician) to the Director of Nursing or registered nurse supervisor.
  12. Perform wound care.
  13. Perform al skills, treatments and procedures competently in accordance to the scope of practice as defined ni the Nurse Practice Act in the state of clinical practice and adheres to the
    policies, procedures and protocols of the agency.
  14. Follow principles of infection control and Universal Precautions. 15. Identify and participate in performance improvement activities.
  15. Use equipment properly and in a safe manner.
  16. Ensure client's rights are adhered to. These include but are not limited to:
    a. the right to privacy and dignity while care is being provided
    b. the right to accept or refuse care
    c. the right to confidentiality of their information

Qualifications:

  1. Graduated from a National League of Nursing accredited practical or vocational school of nursing.
    Unencumbered license as a Practical or Vocational Nurse in the state that care is delivered.
  2. 1 year of nursing experience required; three years nursing experience preferred. Community, home health or medical/surgical experience preferred.
  3. Comply with the state nurse practice act.
  4. Comply with accepted professional standards of practice.
  5. High level of interpersonal skills.
  6. Certified in CPR.
  7. Strong clinical skills
  8. Excellent customer service skils and ability ot deal tactfully with clients, other team members and community.
  9. Treat clients, team members and the public with courtesy, respect and presents a positive public image. 1. Empathy for the needs of the client.
  10. Works as a team member.
  11. Self-directing with the ability to work with little direct supervision.
  12. Licensed driver with automobile that si insured in accordance with the organizations requirements.
  13. Negative TB test or chest x-ray.

Personal Protective Equipment or Physical Demands Environment:

Contact with clients under a wide variety of circumstances. Care provided ni client's living environment with varying situations or ni an institution such as a hospital. May be required to respond to emergency situations. Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.

Personal Protective Equipment:

May include gloves, mask, eye/face protection, and disposable outer covering or lab coat.
Physical Requirements:

  1. Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance si required depending on the ability of the client. Must be able to lift up to 50 pounds. Visual and hearing acuity required.

Travel Requirements:

  • Limited travel as needed for various staff and client care service needs.
  • Up to 10% or as directed based on the needs of the business and attending job required training and conferences.


Work Environment:

  • Office Environment: high-paced office/branch environment; continuous phone use/ringing; talking; various noise levels; interactions with employees and clients; sitting and standing for long periods of time; bending and stooping required; lifting up to 10 lbs. regularly and up to 50+ Ibs. may be required; using a computer for extended periods: typing, staring at a bright monitor or TV screen; using fingers, wrists, and arms; stress on back may occur; possible exposure to prescription drugs, bodily fluids, small appliances for cooking, cooling, and heating foods or securely storing medications.
  • Client's Environment: Unknown environments; bodily fluids, household and human germs; extreme cold or hot temperatures in or outside; light to heavy traffic; inclement weather; human body weights that vary based on client health and lifting as prescribed or needed per plan of care; 50+ Ibs.
