Juvenile To Healing Wellness Court Case Manager

The incumbent works with the youth offenders who have been referred to the Juvenile to Healing Wellness Court (JWC) by the Tribal Prosecutor. Works closely with the JWC Director, youth offender and parents on a corrective case plan developed with the youth and family, approved by the JWC Director and the Juvenile Healing to Wellness Court Activities. Work with youth and family to alleviate negative behavior by the use of the four Traditional and Alternative justice solutions.

ESSENTIAL DUTIES and / or RESPONSIBILITIES include the following :

• Will receive assigned cases from the Juvenile Wellness Court (JWC) Director.
• Will develop individualized Case Plans with each youth and their families and submit such plan to the JWC Director for approval and Judge for review.
• Provides intense case management and monitoring of assigned youth and will work with the parents to ensure recommendations by the JWC Judge are being followed. The Case Manager will participate in the weekly report on the youth's adherence to the Plan activities.
• Monitors the youth's participation at the school; meeting with the school teachers and counselors insuring that the youth is meeting the grade requirements. The Case Manager will also meet weekly (sometimes daily) with the Truancy Officer to ensure attendance is at 100% per case plan.
• Ensures the referral of the youth to specialized services, i.e. alcohol assessment or treatment, is completed. The Case Manager will work closely with the family on transportation issues and if transportation is required, will transport family to scheduled appointments.
• Meets with other tribal youth program staff to keep abreast of youth issues in the community.
• Meets regularly with the WC Director to discuss cases, plan activities, and made recommendation for program improvement.
• Maintains records of activities with each youth assigned to them and make those reports available to the WC Director for grant reporting purposes.
• Works with OJJDP trainers and TA providers.
• Serves as an active member of the Omaha Nation Juvenile Justice Planning Team (ONJJPT) including attendance at all planning sessions, participation in data gathering and training offered specific to the purpose of the WC.
• Submits a monthly narrative report of activities to the WC Director.
• Meets quarterly with the WC Director and judges to discuss program concerns and growth.
• Performs other duties as assigned
