HOME CARE NURSE

Job Summary :

We are looking for a skilled Registered Nurse (RN) to provide patient-centered care in a home setting.

Full-time and part-time positions are available.

Key Responsibilities :

• Conduct in-home patient assessments and develop individualized care plans
• Administer medications, IV therapy, wound care, and other nursing interventions
• Monitor patient health status and adjust care plans as needed
• Collaborate with physicians, therapists, and family members to coordinate holistic care
• Educate patients and caregivers on medication use, chronic condition management, and preventative care
• Ensure compliance with home care protocols and state / federal health regulations
• Provide emotional support and advocacy for patients and families
