Physician Affiliate Group of New York (PAGNY) is adding Physician Assistant - Care Continuum Specialist tothe group at NYC Health + Hospitals / Kings County. Kings County Hospital operates a world-renowned Level I Trauma Center, one of only three in Brooklyn, which serves 2.6 million residents of Brooklyn and Staten Island.The hospital maintains a strong academic affiliation with SUNY Downstate Health Sciences University to maintain its high healthcare delivery standards.

Opportunity Details :

• Patient and family communication and response.
• Abnormal results follow-up and management.
• PCP In Basket coverage.
• eConsult recommendation ordering and follow-up.
• Performing direct (supervised) primary care can be integrated into this role.
• This will be a hybrid position (both remote and in-person) with flexible scheduling options, including evenings and weekends.

Qualifications

Wages and Benefits include :

Annual Base Salary : $131,127

The annual total value of compensation package is estimated at $144,239

Employees are immediately vested in a 3% company contribution of base earnings. No employee match is required.
