We are currently seeking a motivated and experienced Hospital Liaison  to join our team. The Hospital Liaison  plays a critical role in facilitating communication and collaboration between our hospital and external healthcare providers, community organizations, and patients. Hospital Liaison  Responsibilities :

• Serve as the primary point of contact for external healthcare providers referring patients to our nursing home
• Build and maintain relationships with physicians, case managers, social workers, and other healthcare professionals
• Coordinate patient transfers and admissions to our hospital, ensuring a smooth transition of care
• Educate external stakeholders about our services, specialties, and admission criteria
• Conduct outreach activities to promote our programs and services to the community
• Gather and analyze data on referral patterns, patient demographics, and market trends
• Collaborate with internal departments to address patient needs and concerns
