CNA - Med / SURG

Job Title

Primary Purpose : To provide quality patient care by assisting the providers, RN, or LPN. Performs other duties as assigned.

Routine Decision Making : Determine when to report patient concerns and abnormal vital signs to the nurse, determine appropriate supplies necessary to maintain patient care.

Required Knowledge : Medical terminology and normal patient vital signs.

Required Skills and Ability : Organization, problem-solving, computer skills, customer service, written and verbal communication skills.

Unusual Working Conditions : Risk of blood, body fluid contamination. Risk of injury related to poor body mechanics. Risk for injury from aggressive, combative or disoriented patients.

Education and Certification / Registration Required for the Position : High school education required. CNA license, have completed first semester of nursing school program or have an unlicensed assistive personnel equivalency required. BLS required within 90 days of hire.

Age Specific Competencies : May provide care for infants (birth-2 yrs.), pediatric / child patients between (2 yrs-12 yrs.); adolescent patients between 12 to 18 years of age; adult patients between 18 to 64 years; and geriatric patients 65 years and older.

A. Assists in implementation of plan of care for all assigned patients daily. (60% of time)

1. Assists with activities of daily living throughout shift.

2. Assists patients with activity order as prescribed by provider each shift.

3. Obtains vital signs / weights on assigned patients as ordered daily.

4. Reports any abnormal findings to team leader immediately.

5. Assists staff nurses and / or providers on each shift.

6. Ensures patients have fresh water throughout shift, if provider orders allow.

7. Ensures each patient has an identification bracelet upon admission to the unit and upon bands being removed.

8. Updates white boards each shift.

9. Utilizing the 'AIDET' (Acknowledge, Introduction, Duration, Explain, Thank) model, conducts an appropriate introduction, reviews the plan of care, mutually sets attainable patient goals.

10. Bedside report and patient rounding to occur every shift between oncoming and off-going staff.

11. Attends unit staff meetings (3) 80% or greater, (2) 50-80%, (1) less than 50% annually.

B. Maintains a safe and comfortable unit. (25% of time)

1. Keeps patient rooms and hallway free of clutter daily.

2. Ensures alarms on any assigned areas are audible to alert staff, each shift.

3. Removes malfunctioning equipment from the direct patient care area immediately upon discovering a problem.

4. Completes a requisition for repair and sends electronic work request before the end of the shift, pass on any issues to the next shift for follow up and provides a copy to the manager before the end of shift.

5. Utilizes correct patient identification by the hospital approved, two methods, for positive patient identification.

6. Acts rapidly and calmly during emergencies.

C. Demonstrates accountability to the nursing process through documentation. (5% of time)

1. Utilizes only approved abbreviations in patient care documentation each shift.

2. Documentation follows hospital and departmental policies for frequency and content.

D. Develops and promotes performance improvement initiatives. (10% of time)

1. Plans, develops and evaluates self-objectives, annually.

2. Participates in planning ways to solve problems and make improvements in the Medical Surgical unit, quarterly, and as needed.

3. Participates in implementing planned changes and activities to improve the Medical Surgical unit quarterly, and as needed.

Essential Physical Requirements

Type of Work : Sedentary Work : Lifting 10lbs maximum and occasionally lifting and / or carrying such articles as dockets, ledgers and small tools. Jobs are sedentary, if walking and standing are required only occasionally.

Light Work : Lifting 20lbs maximum with frequent lifting and / or carrying of objects weighing up to 10lbs. Even though the weight lifted may be only a negligible amount, a job is in this category when it requires walking or standing to a significant degree.

Medium Work : Lifting 100lbs maximum with frequent lifting and / or carrying of objects weighing up to 25lbs.

Heavy Work : Lifting 100lbs maximum with frequent lifting and / or carrying of objects weighing up to 50lbs.

Very Heavy Lifting : Lifting objects in excess of 100lbs with frequent lifting and / or carrying of objects weighing 50lbs or more.
