Minimum of 2 years of experience as an inpatient Registered Nurse Case Manager.

\nComplete a comprehensive patient assessment, including evaluating patient status to facilitate discharge.

Drive progression during the patient's stay.

\n

Plan for discharge in a manner that expedites the patient’s transition to the next level    of care.

\n

Maintain comprehensive and accurate documentation in EPIC .\n

\n

Ability to identify necessary tasks and follow through to completion without prompting.\n
