Key Responsibilities :

Patient Care :

Provide high-quality, safe, and cost-effective care to patients in post-acute care communities.

Collaboration :

Work closely with interdisciplinary teams, including hospice, palliative care, and home care agencies, to ensure safe transitions of care.

Documentation and Billing :

Ensure timely documentation and billing through our EMR system (Gehrimed), meeting Medicare guidelines and ensuring compliance with all relevant regulations.

Qualifications :

Willing to train new candidates; experienced candidates may be eligible for increased compensation due to prior experience in a post-acute care setting.
