At SSM Health , we are committed to delivering exceptional healthcare services, and our Wisconsin hospice team is seeking a part-time Nurse Practitioner for face-to-face visits and eligibility assessments at our facilities in the greater Madison area. • Provide care in a variety of environments : hospital, SNFs, and some in-home visits
• Schedule
• Part-time : 16 hours / week
• Dayshift Monday-Friday with flexibility
• NO weekends or call!
