Tennessee Licensed Bilingual LPN

A company is looking for a Spanish & English speaking LPN in Tennessee.

Key Responsibilities :

Serve as the primary point of contact for patients enrolled in RPM and CCM programs

Review transmitted patient health data daily, identify red flags, and escalate issues to the clinical team

Provide culturally competent health education in Spanish and English, empowering patients to manage chronic conditions effectively

Required Qualifications :

Active Tennessee RN or LPN license

Fully bilingual in Spanish and English (oral and written)

2+ years of experience in care coordination, telehealth, or chronic disease management

Proficient in EHR systems and comfortable with telehealth technology

Organized, compassionate, and able to thrive in a remote, HIPAA-compliant workspace
