We are seeking a compassionate and dedicated Registered Nurse to join our Hospice Team in the Pottsville, PA and srrounding areas WEEKENDS ONLY . The ideal candidate will have a strong clinical background and a passion for providing exceptional end-of-life care. Our team is committed to providing personalized support and comfort in the familiar surroundings of patients' homes.

Responsibilities : What You’ll Do :

• Provide skilled nursing care to hospice patients in their homes or in a hospice facility.
• Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.
• Administer medication, monitor vital signs, and manage symptoms to ensure patient comfort.
• Educate and support patients and their families regarding end-of-life care and symptom management
• Coordinate care with other healthcare professionals and community resources.
• Offer emotional support, counseling, and education to patients and their families, addressing their unique needs and concerns.
